Guidelines for Online Appointment Requests

• Appointments may be scheduled for no sooner than one week in advance (not for acute or same-day visits).
• Proceed according to the following steps (no exceptions):
     1. Complete and Submit the form.
     2. We will email you with a tentative appointment.
     3. You reply to our email to confirm your appointment.
• Although we will try to hold this time for you, if available appointments  are scarce, another patient able to confirm an appointment may displace your tentative appointment until you confirm. If you do not confirm, we will assume that you changed your mind and intend to cancel the appointment request.
